Students entering 7^th grade in August must have the following to attend:

• A current physical documented within the last 12 months prior to the start date of the new school year. Note, any physical dated prior to August 18, 2007 will not be accepted. All physicals need to be documented on the Florida State form #DH3040.
• Up to date immunizations. Students entering 7^th grade must show proof of immunizations for the Hepatitis series, Tetanus/Diphtheria Booster, a 2^nd dose of MMR Vaccine, and Varicella. The Varicella vaccine is not required if history of the disease is documented. Immunizations must be documented on the Florida State form # 680

Note that schedules will not be distributed without documented proof of the above physical and immunization requirements.
